Laboratory Workup
- • Blood culture (minimum 1 mL) before antibiotics
- • CBC with differential at 6–12 hours of life
- • CRP at 12–24 hours (trend, not single value)
- • Consider lumbar puncture if blood culture positive or clinical deterioration
- • Urine culture only if > 24–48 hours of age or urinary anomaly
Observation & Monitoring
- • Vitals with exam every 4 hours for the first 24 hours
- • Escalate for temperature instability, respiratory distress, or poor feeding
- • Reassess need for antibiotics at 36–48 hours with culture results
- • Document serial exams — observation alone is appropriate for low-risk, well-appearing infants
